Job Description Brookdale is on the lookout for a dedicated Mobile Clinical Services Director! This role is ideal for individuals experienced in clinical operations and familiar with state regulations. A Compact license covering Indiana is required. This is a traveling role, with a schedule of 10 days on followed by 4 days off. Responsibilities Oversee the daily healthcare operations to ensure residents' healthcare needs are met, guaranteeing that care is delivered with respect and dignity. Supervise licensed nursing staff and direct care personnel within the community. Enhance clinical processes until a replacement is designated, and provide support to the newly appointed Health and Wellness Director. Travel within the division as required for management vacancies, start-ups, vacations, etc. Directly supervise community-based licensed nursing staff (LPN/LVN, RN), Med Techs/CMAs (as required by State Regulations), and Lead Resident Care Associates. Supervise Resident Care Coordinators and Supervisors as necessary. Assign and monitor tasks for staff, evaluate performance, recognize achievements, and address concerns as needed. Train and oversee medication administration as per the Nurse Practice Act, including auditing medication administration records. Provide continuous education and training for resident care associates in various settings. Supervise maintenance of resident records and review documentation by caregiving staff. Assess the health and psychosocial status of residents to initiate personalized service plans, proactively manage care, and evaluate service effectiveness. Manage resident healthcare, ensuring families and staff are informed about scheduled appointments. Assist in pre-admission screening for prospective residents, ensuring required documentation is completed. Conduct evaluations of residents in alternative care environments and maintain communication with families. Regularly assess residents' needs, collaborating with other departments to ensure quality care. Document changes in residents’ conditions and communicate updates to executive directors, physicians, and families. Coordinate care for residents receiving home health care, hospice, and other healthcare services. Participate in or lead multidisciplinary meetings on resident care issues, ensuring appropriate involvement from necessary parties. Schedule and ensure follow-ups for in-house ancillary medical services such as doctor visits and lab services. Engage in departmental quality improvement initiatives. Contribute to maintaining a safe environment for staff and minimize Workers' Compensation claims by assessing resident mobility needs and implementing therapy services as necessary. Share on-call and manager-on-duty responsibilities as needed. Support additional requirements and tasks within the division as assigned. Qualifications Education and Experience Education must meet the requirements for obtaining a licensed nursing credential (LPN/LVN or RN). Preferred experience includes nurse management, senior living, or post-acute care. Proficiency in typing and personal computing, specifically in a Microsoft Windows environment, is required. Certifications, Licenses, and Special Requirements LPN/LVN or RN license is mandatory. Physical Demands and Working Conditions Standing and walking are required. Interaction with coworkers, residents, and vendors is necessary. Sitting may be required at times. Weekend, evening, or night shifts may be necessary to ensure coverage. Hand use for handling or feeling objects is required. On-call duties may be required. Reaching with hands and arms is necessary. There may be exposure to communicable diseases and infections. Climbing or balancing may be required at times. There is potential injury from managing resident mobility. Effective communication abilities (talking or hearing) are essential. Exposure to latex is possible. Ability to lift up to 50 pounds is required. Possible exposure to bloodborne pathogens, chemicals, or biological hazards. Risk of injury from falls, burns, or cuts from equipment. Travel may be necessary at times. Vision abilities must be sufficient for job performance. Management/Decision Making The role requires using existing guidelines and procedures to make departmental decisions. Sound judgment and experience are applied to navigate moderately complex issues. Knowledge and Skills Possesses in-depth knowledge of specific skills and a robust understanding of the organization and work environment.
