RESPONSIBILITIES: Provide direct nursing care to residents under supervision, following physician orders. Administer medications and treatments per orders and established facility policies. Monitor and document residents' vital signs and any changes in their condition. Assist residents with activities of daily living (ADLs) as needed. Perform basic wound care and dressing changes, and provide catheter or ostomy care within your scope. Collect lab specimens as ordered by the physician. Document residents' physical and emotional status accurately; report significant changes to the supervising RN or physician. Maintain timely, accurate documentation of care provided and treatments administered. Facilitate effective communication with residents, families/POAs, and the healthcare team about care needs and concerns. QUALIFICATIONS: Valid, unrestricted LPN license in Connecticut. Graduate of an accredited practical/vocational nursing program. Current CPR/BLS certification as required by facility policy. Strong interpersonal and communication skills, with the ability to work well with residents and the care team. Compassionate, resident-centered approach, with excellent organizational skills.
