Job Description & Requirements Specialty: Case Management Discipline: RN Start Date: 04/06/2026 Duration: 15 weeks 40 hours per week Shift: 10 hours, days Employment Type: Travel Care Manager RN – Job Summary Role: Care Manager RN (Care Coordination / Case Management) Location: Acute Care / Hospital setting (Emergency Department possible) License Required: Active California RN License Key Responsibilities: Coordinate patient care and discharge planning throughout the acute care stay. Work closely with physicians, bedside RNs, social workers, and utilization managers to ensure patients move to the appropriate level of care. Help prevent unnecessary admissions or readmissions through proper care coordination. Facilitate communication across the care continuum and coordinate resources for patients. Collaborate with patients, families, interdisciplinary teams, payers, and community providers. In the Emergency Department, manage complex clinical and social cases to avoid unnecessary hospital admissions.
