As a PRN Licensed Occupational Therapist, you will play a vital role in enhancing the lives of our residents by evaluating their functional abilities and creating tailored treatment plans aimed at fostering independence in daily activities. You will engage in hands-on therapy, work collaboratively with interdisciplinary teams, and deliver exceptional rehabilitation services that align with our residents' personal goals. Key Responsibilities Evaluate residents' functional, cognitive, and psychosocial needs through standardized assessments. Create and implement customized treatment plans to enhance daily living skills, mobility, and overall safety. Deliver therapeutic interventions, including adaptive equipment training, therapeutic exercises, and functional retraining. Educate residents, families, and staff on effective strategies to achieve rehabilitation goals. Accurately document evaluations, progress notes, and discharge summaries in compliance with regulatory and company standards. Collaborate with physical therapists, speech-language pathologists, nurses, and other team members to ensure coordinated care. Maintain compliance with all applicable state and federal regulations. Qualifications Current licensure as an Occupational Therapist (OT) in your state of practice. Graduation from an accredited Occupational Therapy program. Experience in long-term care, skilled nursing, or rehabilitation is preferred. Excellent communication, documentation, and organizational skills.
