The Wellness Program Coordinator (WPC) plays a vital role in this mission by leading wellness assessments, facilitating health education, and connecting residents with essential services. The WPC is responsible for coordinating care and services to help residents live safely and well for as long as possible. This position is based on-site at an affordable housing community in Boston and requires fluency in Cantonese or Mandarin, as our resident population primarily speaks these languages. Position Responsibilities: Conduct comprehensive wellness assessments to identify resident needs and goals, creating wellness plans that encompass physical, social, cognitive, spiritual, and mental health. Collaborate with primary care physicians, mental health professionals, and hospitals to ensure continuity of care. Work with housing staff to develop engaging programming tailored to resident needs. Train and educate housing staff on recognizing and reporting changes in resident conditions. Assist residents in organizing important healthcare information, advance care planning, and emergency contacts. Serve as a key resource to at-risk residents, promoting adherence to health and wellness activities. Foster relationships with payers serving the senior population. Establish effective communication systems to share vital information regarding resident health and activities. Encourage self-care among residents through personalized coaching, helping them set and achieve their health goals. Partner with local service providers and make referrals as appropriate, such as for rehabilitation and dementia services. Lead and participate in care planning meetings held on-site. Assess and refer residents with dementia for specialized care as needed. Collaborate with the Nurse Care Manager to plan and implement evidence-based health programs. Work with local hospitals and home care services to facilitate safe transitions and discharges. Support residents in transitioning to different levels of care, involving families in meetings and discussions as necessary. Advocate for residents and act as a liaison for their families. Monitor and report on program outcomes and effectiveness. Qualifications: Master’s degree in social work or a related field with 1-2 years of case management or care coordination experience in aging services preferred; a bachelor’s degree with 3-5 years of experience is required. Genuine compassion and desire to work with the senior population. Exceptional organizational and interpersonal skills, capable of managing multiple projects efficiently. Professional, proactive, and results-driven attitude. Positive and optimistic disposition with strong communication skills. Willingness to learn and adapt to change. Ability to document clearly using AASC Online software. Proficiency in computer applications including Windows, Word, PowerPoint, and Excel. Some travel in the Boston metro area may be required. Certification in dementia care is preferred. Fluency in Cantonese or Mandarin is required. Salary Range: $52,913.00 - $79,370.00
