Job Details Position: Travel Physical Therapist (PT) - Home Health / In-Home Physical Therapy Location: Novato, CA 94945 (Marin County, Northern California; near San Francisco) Assignment Type: 13-week travel contract (Home Health setting) Start / End Dates: March 30, 2026 - June 29, 2026 Shift / Hours: Day shift, 8-hour days (08:00 - 17:00); 40 hours/week Estimated Weekly Pay: $2,210 - $2,370 Patient Population: Adults, Geriatrics - home health PT caseload (~5 visits/day) Interdisciplinary Support: PT, Nursing, Social Services, Case Management Job Requirements Graduate of an accredited physical therapy program (DPT or equivalent) Active Physical Therapist (PT) license in California required at time of submission BLS (Basic Life Support) certification from AHA or American Red Cross required Valid Class C driver's license and current automobile insurance Minimum 5 years recent, relevant experience as a Physical Therapist in a home health or hospital setting (home health PT experience preferred) Epic EMR experience preferred (clinical documentation in Epic) Travel PT experience preferred Two professional references from the last year required and 7-year work history verification Responsibilities Perform comprehensive in-home PT evaluations and develop individualized treatment plans for adult and geriatric patients Provide evidence-based interventions including therapeutic exercise, gait and balance training, functional mobility, transfers, and caregiver/patient education in the home environment Assess home safety, recommend adaptive equipment, and identify environmental barriers to safe mobility and discharge Complete OASIS documentation and maintain accurate, timely clinical records using Epic EMR Collaborate with interdisciplinary teams (PT, nursing, social services, case management) to support continuity of care, discharge planning, and optimal patient outcomes Participate in re-assessments, transitions of care, and quality improvement initiatives; stay current with professional practice developments Why Novato, CA & California Physical Therapy Jobs Novato, located in scenic Marin County, blends suburban comfort with outdoor adventure - hiking and biking trails, local wineries, and a welcoming community.
