Shift Details: Friday 5pm to Monday 8am Role Overview In this crucial role, the Afterhours RN Care Manager is responsible for assessing patient and family needs outside regular hours, ensuring adherence to best clinical practices and hospice protocols. Your responsibilities will involve direct patient care, including admissions, routine visits, and urgent calls, while collaborating effectively within the Interdisciplinary Group (IDG) to ensure quality care and communication. Core Responsibilities Deliver high-quality hospice services to eligible patients and their families. Provide in-person patient care, including assessments and visits during designated shifts. Conduct comprehensive assessments focused on the physical, emotional, psychosocial, and spiritual needs of patients. Participate in the IDG process by reviewing charts, updating care plans, and maintaining communication. Collaborate with physicians and team members to develop and modify the Hospice Plan of Care. Educate families about disease progression, symptom management, and hospice philosophy. Promptly report significant patient condition changes to relevant healthcare professionals. Ensure accurate clinical documentation is maintained, including care plans and progress notes. Assist in rescheduling and triaging visits as necessary. Engage in scheduling modifications and support administrative compliance activities. Uphold infection control standards consistently. Offer guidance to field staff and conduct supervisory visits as required. Participate in meetings, training, and after-hours updates. Maintain expertise in pain and symptom management in hospice care. Foster a culture of compassion, professionalism, and teamwork in line with the organization's values. Undertake additional tasks as assigned. Qualifications Active RN license in the state of practice. Graduation from an accredited nursing program. A minimum of one year of nursing experience in settings such as acute care, oncology, long-term care, hospice, or home health. Compliance with state-specific requirements is mandatory. Licenses and Certifications Current RN license. CPR certification (in-person). Valid driver's license, reliable transportation, and current auto insurance. Essential Skills and Competencies Comprehensive knowledge of hospice care, Medicare/Medicaid regulations, and best practices. Expertise in conducting thorough patient assessments and formulating care plans. Strong communication skills to collaborate with patients, families, team members, and physicians effectively. Ability to manage time and priorities in a dynamic environment. Proficiency in EMR systems (HomeCare HomeBase preferred), Microsoft Office, and other mobile technologies. Commitment to maintaining confidentiality and delivering quality outcomes. Leadership abilities to support and mentor field staff.
