Job Description: We are seeking a dedicated Clinical Nutrition Specialist to join our team, focusing on providing exceptional nutrition care through evidence-based practices. This role involves 32 hours per week in Roslindale's Long-Term Care (LTC) facility and 8 hours per week in Orchard Cove, Canton, LTC. Position Summary: The Clinical Nutrition Specialist will play a vital role in delivering patient-centered nutrition care by applying the Nutrition Care Process. Your responsibilities will include nutrition screening, assessment, diagnosis, intervention, care planning, monitoring, evaluation, and supporting transitions of care. Collaboration with the interdisciplinary team is critical to achieving the best patient outcomes. Additionally, you will have the opportunity to contribute to quality improvement projects, develop educational materials, assist in onboarding new staff, and mentor dietetic interns. Core Competencies: Thoroughly document patients' nutrition status in the electronic medical record using the ADIME format. Utilize critical thinking skills to assess and determine patients' clinical nutrition needs. Understand culinary and menu operations to ensure patient-centered care regarding diet orders, food allergies, sensitivities, and preferences. Provide attentive and empathetic service to patients and their families by understanding their priorities. Be prepared to mentor dietetic interns following the Nutrition and Dietetics Preceptor Training Course guidelines. Position Responsibilities: Conduct initial nutrition assessments and follow-ups in the Rehabilitation Services Unit (RSU) and Medical Complex Unit, ensuring communication with healthcare proxies, and interdepartmental meetings for collaborative care. Complete required documentation promptly for RSU patients. Perform annual, monthly, and quarterly nutrition assessments for Long-Term Care patients and maintain effective communication with all relevant parties. Employ validated tools and current Malnutrition Guidelines to assess patients, including the use of hand grip strength and other assessments. Identify and prioritize high-risk patients based on guidelines. Track and document calorie counts and weight records, addressing significant changes with the team as needed. Collaborate in setting patient-centered short-term and long-term goals based on the Age-Friendly Health Systems framework. Act as a mentor and resource for culinary, nutrition, and medical teams, providing education suited to various learner needs. Conduct meal rounds to evaluate meal adequacy, adherence to prescribed diets, food quality, and patient satisfaction. Give feedback on menu items and special food requests at menu meetings. Ensure accurate management of patient diet orders using Computrition software. Manage individualized meal and snack requests, communicating changes to culinary staff. Perform audits of Country Kitchens to ensure compliance with dietary guidelines. Assist Nutrition Assistants with educational needs. Pursue ongoing professional development to maintain skills and licensure requirements. Navigate professional resources effectively including diet manuals and evidence-based libraries. Follow professional standards and ethical guidelines set forth by the Academy of Nutrition and Dietetics. Contribute to departmental quality improvement initiatives and assist in policy development as needed. Undertake additional departmental duties as required. Qualifications: Bachelor's Degree in Nutrition; a Master's Degree is required for those registered in 2024 or later. Must be a Registered Dietitian (RD) and Licensed Dietitian/Nutritionist (LDN). Preferred experience in healthcare or long-term care settings. Strong computer skills are essential. Excellent communication skills are necessary for success in this role.
