• OCCUPATIONAL THERAPIST (OT)Location: Kingsport, TN Facility: Wexford House• Join Our Team! About the Facility Ahava Therapy Services is seeking a Full-Time OT to join our in-house rehabilitation team at Wexford House, a 173-bed skilled nursing facility offering short- and long-term rehabilitation. Be part of a collaborative therapy team under outstanding leadership, where an Occupational Therapist can deliver high-quality care and make a meaningful difference in patients' lives. • About the Location: Kingsport, TN Kingsport, TN, is known for its blend of affordable living, abundant outdoor recreation, strong community feel, and good schools, all set within the scenic Appalachian Mountains, offering a small-town vibe. It's praised for its low traffic, family-friendly atmosphere, four seasons, and convenient location in the Tri-Cities region. ⚙ Responsibilities Review clinical information and evaluate its impact on the assessment and treatment process. Conduct comprehensive functional assessments using standardized procedures. Develop and implement individualized treatment plans with achievable functional goals. Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly. Revise treatment plans based on patient performance and progress. Supervise and guide therapy assistants, ensuring proper treatment techniques and the delivery of care. Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover. Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes. Assist with discharge planning and maintain knowledge of community resources. Participate in care conferences, meetings, and quality improvement initiatives. Maintain clinical competence through continuing education opportunities. Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines. Support the company's mission, vision, and professional expectations. Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.
