This role provides day-to-day clinical leadership, ensures adherence to physician orders and plans of care, supports regulatory compliance with Medicare and Texas HHSC requirements, and serves as a key resource for clinical staff and agency leadership.Key Responsibilities & TasksClinical Oversight & ManagementProvide clinical oversight of all home health services delivered by field cliniciansEnsure patient care is delivered in accordance with physician orders, plans of care, and professional standardsReview and approve clinical documentation, including assessments, visit notes, and care plansMonitor patient outcomes and address clinical issues or variances in careServe as the clinical escalation point for patient care concernsStaff Supervision & Clinical SupportSupervise, mentor, and support field clinicians (RNs, LVNs, therapists, aides, etc.)Conduct clinical supervision, case reviews, and performance evaluationsProvide ongoing clinical education, coaching, and corrective action as neededEnsure staff competencies, licensure, certifications, and required training remain currentParticipate in hiring, onboarding, and orientation of clinical staffCare Coordination & Interdisciplinary LeadershipCoordinate care across the interdisciplinary team, including nursing, therapy, social work, and physiciansLead or participate in interdisciplinary team meetings and case conferencesSupport transitions of care (hospital to home, recertifications, discharges)Ensure timely communication of care plan updates and changesDocumentation & Regulatory ComplianceEnsure compliance with Medicare Guidelines of Participation, Texas HHSC regulations, and agency policiesReview OASIS assessments for accuracy, completeness, and timelinessEnsure timely completion of physician orders, recertifications, and discharge documentationPrepare for and participate in surveys, audits, and regulatory reviewsAddress deficiencies, develop plans of correction, and monitor compliance improvementsQuality Improvement & Risk ManagementMonitor quality indicators, outcomes, and adverse eventsIdentify trends, risks, or gaps in care and implement corrective actionsOversee incident reporting, investigations, and follow-up actionsParticipate in Quality Assurance and Performance Improvement (QAPI) activitiesSupport infection control and patient safety initiativesScheduling & Operational SupportCollaborate with administrative staff to ensure appropriate clinician coverage and visit frequenciesAssist with workload distribution based on patient acuity and staffing levelsSupport timely start-of-care, recertifications, and dischargesCommunicate staffing needs or operational challenges to leadershipPatient & Family EngagementAddress escalated patient or family concerns and complaintsEnsure patient and caregiver education is provided and documentedPromote patient satisfaction and continuity of careQualificationsRequiredActive Registered Nurse (RN) license in the state of TexasMinimum 2-4 years of nursing experience, with home health experience requiredPrior experience in a supervisory or management role preferredStrong knowledge of Medicare CoPs, Texas HHSC regulations, and home health operationsCurrent BLS/CPR certificationPreferredOASIS and PDGM experienceEMR proficiencyLeadership or management trainingBilingual (English/Spanish preferred)Physical & Work RequirementsAbility to work full-time in an office environmentAbility to sit, stand, and use a computer for extended periodsOccasional travel to patient homes or field visits as neededAbility to lift up to 25 lbsCore CompetenciesClinical leadership and decision-makingRegulatory compliance and documentation accuracyStaff supervision and developmentQuality improvement and risk managementCommunication and organizational skillsBenefits:Health & Dental insurance, Paid Time Off, Holidays, Bonus/Incentives, 401(K)
