RESPONSIBILITIES: Provide compassionate nursing care to residents under the supervision of an RN or physician orders. Administer prescribed medications and treatments, ensuring adherence to facility policies. Record and monitor residents' vital signs and changes in condition. Assist residents with daily living activities (ADLs) including bathing, dressing, and mobility. Perform basic wound care and manage dressing changes as per facility policy. Collect specimens for lab testing as instructed. Observe, document, and report on residents' physical and emotional status to the supervising RN or physician. Maintain accurate and timely documentation of all care provided in medical records. Effectively communicate with residents, families, and the healthcare team regarding care needs and concerns. Adhere to infection control protocols and facility standards of care. QUALIFICATIONS: Valid and unrestricted LPN license in Connecticut. Graduate from an accredited practical/vocational nursing program. Current CPR/BLS certification as required by facility policy. Excellent interpersonal and communication skills; capable of fostering positive relationships with residents and the care team. Compassionate approach with strong organizational and time-management abilities. Experience in a long-term care or skilled nursing environment preferred.
