Shift: Day Shift (8:00 AM – 8:00 PM) Job Overview Advocate Health Southeast is seeking a Travel Physical Therapist (PT) for a Home Health assignment in Albemarle, North Carolina. The PT will provide in-home therapy services to adult patients recovering from orthopedic procedures, chronic illnesses, and functional mobility limitations. This role is ideal for a therapist experienced in home health, OASIS documentation, and independent patient care, with strong communication skills and the ability to travel between patient homes within the service area. Key Responsibilities Provide in-home physical therapy evaluations and treatments Develop and implement individualized care plans to improve patient mobility and independence Perform gait training, balance training, and functional mobility therapy Utilize non-pharmacological pain management techniques Monitor patient progress and modify treatment plans as needed Recognize early signs and symptoms to help prevent hospital readmissions Document patient care using HomeCare HomeBase EMR Communicate effectively with physicians and interdisciplinary care teams Patient Population Adult patients Common conditions include: Chronic diseases Orthopedic conditions and post-surgical recovery (hip/knee replacements) Falls and generalized weakness Congestive Heart Failure (CHF) COPD Occasional stroke patients Requirements Experience Minimum 2 year of Home Health experience Experience with OASIS-E documentation Experience with HomeCare HomeBase EMR (required) Licensure & Certifications Active Physical Therapist State License BLS Certification Valid Driver"s License and proof of insurance Work Details Average Visits: 6 visits per day Weekly Requirement: 30 visit equivalents per week Average Driving Distance: 60–80 miles per day Mileage: Reimbursed weekly Service Areas: Albemarle, Badin, Harrisburg, Mt.
