Speech Language Pathologist

CareOne Home Health is seeking a Speech Language Pathologist to add to our growing team of highly-regarded health professionals. The Speech Language Pathologist is responsible for speech therapy services for patients in San Joaquin County and surrounding communities. This is a part time or per diem opportunity. Minimum 1 year(s) experience as a working SLP 1 year home health experience preferred Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook) Experience with Kinnser (WellSky) EMR preferred Demonstrates good verbal and written communication, and organization skills Possesses and maintains current CPR certification Primary Responsibilities: Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function. Ensuring coordination of care with other team members and marketing team Identifies functional speech defects and establishes a plan of care to improve patient's function. Administers speech therapy program utilizing specialized therapeutic technique and/or equipment. May supervise home health aide in procedures directly related to the speech-language therapy plan of care Performs other duties as required to facilitate the delivery of speech language pathology services.
