Per Diem RN Case Manager - Pediatric Home Health CareGivers America - NEPA, a Medicare-certified Home Care company, is seeking a compassionate and skilled Registered Nurse to join our team as a Per Diem Pediatric RN Case Manager. As a Pediatric RN Case Manager, you will:Complete comprehensive patient assessments, including OASIS data collection, within established timelines Collaborate with physicians, healthcare professionals, patients, and families to develop individualized pediatric care plans Provide direction and supervision to LPNs and Home Health Aides to ensure quality care Monitor clinical documentation for accuracy, timeliness, and regulatory compliance Communicate regularly with patients' families and care teams to coordinate services Participate in quality improvement initiatives and staff education Promote a safe and respectful care environment for patients and colleagues This role is a great fit for nurses who have:Active PA RN License (Required) - BSN Preferred 2+ years of professional nursing experience (pediatric or home health experience preferred) Strong leadership, communication, and organizational skills Ability to manage multiple priorities and work independently Valid driver's license, and current CPR certification We Live and Lead by Our Core Values...
