Our client is currently seeking a Home Health Physical Therapist (PT) - PRN! Home Health Physical Therapist (PT) - PRNPer Diem Territory: Philadelphia, Montgomery, Bucks, Delaware & Chester Counties Role Overview:This PRN Physical Therapist position focuses on delivering 1:1 skilled home-based rehabilitation to patients recovering from surgery, illness, injury, or hospitalization. As part of an ACHC-accredited, Medicare-certified agency, you'll support patients in regaining independence while working with a flexible schedule and a territory-based caseload. Key Responsibilities Conduct comprehensive PT evaluations in patients' homes, assessing mobility, strength, balance, and functional limitations. Develop and implement individualized treatment plans tailored to patient goals and clinical needs. Provide therapeutic interventions focused on mobility training, strengthening, gait training, balance improvement, and fall prevention. Monitor and document patient progress , adjusting care plans as needed to ensure safe and effective outcomes. Educate patients and caregivers on home exercise programs, safe mobility techniques, and strategies to support recovery. Maintain accurate, timely documentation in the agency's EMR system. Collaborate with the interdisciplinary care team , including nurses, physicians, and other clinicians, to coordinate high-quality patient care. Qualifications Active Pennsylvania Physical Therapist license in good standing. Home health experience preferred , but motivated new clinicians are welcome. Strong clinical assessment, communication, and patient-education skills. Ability to work independently and manage a home-based caseload. Reliable transportation for travel between patient visits across assigned counties. Position Details PRN / Per Diem with visits assigned based on your availability and preferred territory.
