Behavioral Health Program Nurse Care Manager / Certified Community Behavioral Health Clinic (CCBHC), New Haven, CT and Hartford CT sites Teams with the CCBHC Project Director in the implementation of a large, cutting-edge behavioral health and health initiative at Catholic Charities. Provides care coordination to clients and supervises project staff. Teams with Project Director to provide direction to treatment teams. Ensures strengths-based, person-centered, quality care is provided in a culturally and linguistically meaningful therapeutic environment for all clients seeking care. GENERAL DUTIES AND RESPONSIBILITIES:Responsible for teaming with the Project Director in implementing a behavioral health and health initiative. Co-leads project integrated care teams composed of clinical leaders, psychiatry, case managers, medical assistant, as needed. Provides integrated care coordination to a patient panel of CCBHC clients, ensuring linkage to community primary and community specialty care. Monitors clients with chronic medical conditions for further identification and linkage to appropriate health care services. Ensures client data collection and EMR entry on blood pressure, body mass index and lab results on a scheduled basis. Collaborates with local primary care providers, including local Federally Qualified Health Care Clinics; with patient consent, provides patient information to primary and specialty care providers to manage patient care; works with evaluator and data team to measure client outcomes. Conducts and reviews health care screenings and assessments. Ensures client linkage to primary and specialty care as needed, working in collaboration with project staff. Supervises and/or provides direction to case managers and medical assistant regarding client health issues. Delegates assignments and monitors effectiveness to ensure best practices within the program and compliance with operational plans and the CCBHC scope. Other duties as assigned. QUALIFICATIONS: Bachelor's Degree in Nursing/RN license with 1-2 years of related experience in behavioral health or integrated care, or Licensed Practical Nurse (LPN) with 2-4 years of related experience including staff supervision. Strong belief and commitment to integrated care models; Knowledge, experience and sensitivity to working with clients with behavioral health, addiction and co-occurring disorder. Exercises good judgment and decision-making. Ability to deal with sensitive issues with diplomacy and discretion. Must be able to multitask with strong organization skills. Knowledge and experience with evidence-based practices in behavioral health and primary care. Commitment to using data to drive decision making and rapid cycle change in programming. Ability to use Microsoft Office Suite and Electronic Medical Record Software and Reporting. Excellent interpersonal skills and able to communicate both verbally and in written form. Ability to maintain a professional demeanor under pressure and operate with a keen sense of urgency and commitment to quality. Bilingual (Spanish-speaking) preferred.
