Responsibilities As a Licensed Practical Nurse (LPN), you will deliver and supervise patient care through assessment, diagnosis, planning, implementation, and evaluation. Essential Duties and Responsibilities Provide direct patient care to infants, children, and adolescents in their homes based on the physician's active care plan. Deliver services according to physician orders while adhering to state, federal, and national regulations under the supervision of clinical staff and the agency Administrator. Conduct comprehensive patient assessments. Administer prescribed medications, treatments, and therapies as ordered. Document clear, concise, and accurate clinical notes in accordance with the plan of care. Notify the RN Case Manager of any changes in the patient's condition. Educate patients and their families regarding diagnosis, treatment, and prevention. Collaborate with other professionals to ensure high-quality patient care. Use appropriate infection control procedures and safety protocols. Comply with all company policies, practices, and procedures. Perform other duties as assigned. Qualifications Education and Work Experience Requirements Current state Licensed Practical Nursing (LPN) license. One (1) year of home health experience preferred. One (1) year of pediatric experience preferred. Current CPR certification. Other Qualifications and Skills Must have reliable transportation to travel to the office and patients' homes. Physical Demands/Working Environment Regularly requires bending, reaching, standing, stooping, sitting, twisting, talking, and hearing. Ability to push and pull 50 pounds (position patient, move equipment, etc.). Ability to support and lift weights up to 50 pounds (ambulate patient) and assist with weights of more than 100 pounds. Carry equipment and supplies as necessary. Ability to see information up to 24 inches away (monitors, computer screens, etc.).
