Description RN Case Manager | Home Visits, Lakeshore Area | Per diem Join Our Team as an RN Case Manager for Home Visits in beautiful downtown Holland! Atrio Home Care, the partnership of Holland Home and Resthaven, is seeking a compassionate RN Case Manager to join our Lakeshore Team on a per diem basis. This position will involve providing direct care to clients in the Ottawa and Allegan Counties. Currently, our greatest need is in the South Territory including, Saugatuck, Fennville, Allegan, and Hamilton. Responsibilities Perform initial comprehensive assessments and deliver compassionate care to clients Collaborate with physicians and other care team members to develop a Plan of Care Promotes client and family/caregiver independence through counseling and education Coordinate client discharge planning Maintain timely documentation using our Matrix EMR system using iPads Orders medication and supplies for clients' home delivery Participates in on-call rotation Location: 35-mile radius in Ottawa and Allegan Counties Schedule/Shift Per diem Day shift 8-5pm, with flexibility to craft your schedule to help you maintain a healthy work/life balanc Compensation Our compensation model includes bonuses for high-quality client outcomes and efficient case management Premium pay for weekend & holiday visits Qualifications Licensed RN in Michigan; BSN preferred (Bachelors preferred) Valid Michigan driver's license, proof of auto insurance, and reliable transportation for home visits 1-2 years of acute care or skilled nursing experience; prior certified home health experience preferred Passion for delivering high quality care to older adults Benefits Paid Time Off (PTO) Medical, Dental, Vision, STD, LTD, Life Insurance Coverage Generous 403b match with access to a professional Financial Planner at no cost Why work for Atrio?
