This is a 100% outpatient role focused on building long-term patient relationships while delivering high-quality preventive and chronic care. Compensation $120,000 $130,000 base salary Up to $5,000 annual productivity & quality bonus Schedule Choose 4 or 5 regular-length days per week No nights or weekends Built-in flexibility to support lifestyle balance What You ll Do Provide comprehensive primary care including wellness visits, acute care, and chronic disease management Perform patient assessments, exams, and diagnoses using evidence-based guidelines Prescribe and manage medications and treatment plans Order and interpret labs, imaging, and diagnostic studies Educate patients on preventive care, lifestyle, and disease management Coordinate care with physicians, nurses, and care teams Document in EMR and support population health & quality initiatives What We re Looking For PA or NP degree from an accredited program Board Certified (Family Medicine or Internal Medicine) Active New York license DEA (or ability to obtain) BLS/ACLS Clean malpractice history Benefits Package CME: $1,500 + 3 paid CME days PTO: 2 weeks + 6 paid federal holidays 401(k): 6% employer match Malpractice insurance Medical, Dental & Vision Insurance Life Insurance AI scribe options Supportive, collegial culture across 35+ locations How to Apply Interested in learning more?
