Virtual Nurse Practitioner

As a Nurse Practitioner at CareBridge, you will have the opportunity to shape the lives of individuals in need while working in a supportive virtual environment, with the flexibility to balance your professional and personal life. Your Impact: Deliver primary and urgent health care via telephonic and tele video modalities to patients receiving home and community-based services through state Medicaid programs and dual eligible members. Collaborate with a multi-disciplinary team to create and execute comprehensive care plans for adult patients with chronic and complex conditions. Conduct thorough assessments and implement tailored treatment plans that align with patients' health goals and conditions. Identify and address gaps in patient care. Meet the holistic needs of patients through supportive communication and resources, while involving family members in care discussions. Provide education on medication management, illness progression, nutrition, and health maintenance to empower patients and families. Coordinate with clinical team members and other healthcare providers to ensure timely and effective patient care. Maintain accurate medical records and documentation in compliance with regulatory and company standards. Engage in ongoing professional development as required. Prescribe medications within the limits of state regulations. Minimum Requirements: Master’s degree in Nursing. Active national Nurse Practitioner certification. Valid, unrestricted Family or Adult Nurse Practitioner license in Ohio. Experience working with Electronic Medical Records (EMR). Minimum of 2 years managing complex care cases. Preferred Qualifications: Active Medicaid number in Ohio is highly desirable. Possession of DEA registration or eligibility is preferred. Experience with complex care management for patients with developmental disabilities and chronic illnesses is strongly preferred.
