RESPONSIBILITIES: Provide direct nursing care under the direction of an RN or physician orders. Administer medications and treatments as prescribed and according to facility policy. Monitor residents' vital signs and condition, documenting any changes. Assist residents with daily activities as needed, including bathing, dressing, and mobility. Perform wound care and other basic procedures within your scope of practice. Collect specimens for lab tests as required. Document care provided and resident observations accurately and timely. Communicate effectively with residents, families, and the healthcare team about care needs. QUALIFICATIONS: Valid and unrestricted LPN license in Connecticut. Graduate from an accredited practical/vocational nursing program. Current CPR/BLS (or equivalent) certification as necessary. Excellent communication and interpersonal skills. Compassionate, resident-focused approach with strong organizational abilities. Experience in a long-term care or skilled nursing environment preferred.
