Your responsibilities will include: Conducting patient visits and coordinating care among various healthcare disciplines Serving as the primary contact for all care providers involved in a patient's episode of care Consulting with physicians and office staff regarding patient care details Performing accurate OASIS collections and maintaining an up-to-date medication profile Ensuring timely reporting of lab values to physicians Participating in weekly case conferences and monthly case manager meetings Qualifications: To be considered for this role, candidates must meet the following qualifications: Current Registered Nurse (RN) licensure in the state of employment At least one year of clinical nursing experience is preferred Proficient knowledge and skills in contemporary nursing practices Possession of a valid state driver's license and automobile liability insurance Additional Information: Enhabit Home Health & Hospice is proud to be an equal opportunity employer.
