Field Nurse Practitioner (Home & Community-Based Care)We are seeking a Field Nurse Practitioner to provide preventive and primary care services directly in the communities where patients live. This role focuses on in-home visits, skilled nursing facilities, and community-based pop-up clinics, delivering care to patients who may face barriers to accessing traditional healthcare settings.This is an excellent opportunity for a provider who enjoys autonomy, fieldwork, and building meaningful connections with underserved and vulnerable populations!The Nurse Practitioner will perform comprehensive medical assessments and preventive screenings for patients across a variety of settings.
