This role involves providing personalized, one-on-one physical therapy services to patients in the comfort of their own homes. The ideal candidate will be a self-starter with a strong clinical background and a passion for helping patients regain function, mobility, and independence. This position is open to both local and traveling Physical Therapists. Responsibilities: Patient Assessment and Care Planning: Conduct comprehensive initial and ongoing patient assessments to develop individualized, goal-oriented treatment plans in collaboration with the patient, their family, and their physician. Therapeutic Interventions: Administer a wide range of skilled physical therapy treatments and interventions, including therapeutic exercises, manual therapy, gait training, and the use of assistive devices. Patient and Family Education: Educate patients and their caregivers on proper body mechanics, home exercise programs, safety precautions, and disease management to promote self-care and prevent injury. Documentation and Communication: Maintain accurate, timely, and thorough electronic health records (EHR) of all patient visits and progress. This includes detailed OASIS documentation for Medicare and other payers. Interdisciplinary Collaboration: Work closely with the entire healthcare team-including physicians, nurses, and other therapists-to ensure a coordinated and holistic approach to patient care. Supervision: Provide supervision and guidance to Physical Therapist Assistants (PTAs) as required by state regulations. Qualifications: Doctor of Physical Therapy (DPT) degree from a program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE). Current Physical Therapist (PT) license in the Commonwealth of Massachusetts. Minimum of one (1) year of recent clinical experience as a PT, with prior home health or outpatient experience strongly preferred. Proficiency in electronic health record (EHR) systems and experience with OASIS documentation. Current BLS/CPR certification. Strong communication, organizational, and time-management skills. Ability to work independently in a patient's home environment. Valid driver's license and reliable transportation. Shift: M-F -8am - 5pm, 40 hours per week. 13 weeks 30 units HH required HCHB highly desired No guarantee, FT available Qualifications: HCHB or the online training & HH experience required Skills Current State License as a Physical Therapist - Required Proof of current CPR certification - Required Current driver's license. Must have reliable transportation and insurance - Required 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required Previous home care experience - Preferred Education Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required
