Residency must be outside a 50-mile radius of the facility to qualify. Ability to adhere to requested time off policies, with no more than 5 days of time off allowed during the assignment; all time-off requests must be submitted and approved upfront. Commitment to a full contract duration with reliable attendance and performance.
