We're hiring dedicated, dependable Registered Nurses (RNs) for permanent, ongoing positions - not PRN or occasional work. This role is ideal for RNs who want steady, meaningful engagement with clients and are ready to provide specialized care, including Trach Care and Tube Feeding (training provided). Our 24/7 support model ensures you're never alone in the field and always backed by a responsive, reliable office team. Key Responsibilities: Provide Trach Care safely and effectively, following training protocols and monitoring for any complications. Administer Tube Feedings accurately and monitor for tolerance, side effects, and response. Monitor clients' overall health, vital signs, and respiratory status, reporting changes to the supervising healthcare team. Maintain accurate and timely documentation of all care provided. Follow individualized care plans to ensure clients' safety, comfort, and well-being. Communicate effectively with clients, families, and the healthcare team regarding care and progress. Adhere strictly to all infection control, safety, and hygiene protocols. Provide compassionate, patient-centered care during 2-hour visits, times TBD, delivering skilled nursing support. Job Responsibilities Provide client-facing care as assigned Provide assistance with daily tasks as directed in the client's personalized plan of care, including but not limited to: feeding, meal preparation, linen changes, light housekeeping, providing transportation to doctor appointments, outdoor activities, and shopping Provide companionship, personal and hygiene care as directed in the client's personalized plan of care, including but not limited to: bathing, grooming, oral care, dressing, undressing, toileting activities, and assisting with measurements, if necessary Assist clients with active and passive mobility work defined by the plan of care, and document observed changes to the Director of Nursing Assist with various office / administrative tasks Participate in office and field on-call as needed Assist the scheduling team with assessing the competency of staff to promote quality, continuity, and safety of care for the agency's clients Train and educate field staff as needed to ensure services are provided in accordance with brand standards Complete new caregiver introductions for home care cases and staffing clients Participate in the intake of new clients and onboarding of new employees Assist office staff with coordinating and executing employee and client appreciation activities throughout the year Assist the office and staff with other duties as needed Adhere to the policies, procedures, and protocols of the agency of employment Adhere to HIPAA laws and ensure confidentiality and security of the client's medical information Job Requirements High school diploma or GED Minimum one (1) year of documented home care assistance and/or direct care experience at an assisted living or skilled nursing facility, etc., is preferred Preferred: Successful completion of a nursing assistant or certified home health aide program Certified CPR and negative TB skin test or chest x-ray Must have a valid driver's license and auto insurance (proof of insurance is required) Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds Self-directing with the ability to work with little direct supervision Ability to adhere to HIPAA and ensure confidentiality and security of the client's medical information Ability to read, write, speak, and understand English and communicate effectively Up-to-date vaccinations including COVID-19, Influenza, and Hepatitis B Ability to travel within Northeast Ohio territory, up to 60 miles from Middleburg Heights in some cases Must have flexible availability Monday through Friday and some weekends BrightStar Care - Cuyahoga West serves the west side of Cleveland and surrounding areas (Cuyahoga, Lorain, Medina counties), including the cities of Avon, Avon Lake, Bay Village, Berea, Brecksville, Broadview Heights, Brooklyn, Brookpark, Brunswick, Fairview Park, Independence, Lakewood, Middleburg Heights, North Olmsted, North Royalton, Olmsted Falls, Olmsted Township, Parma, Parma Heights, Rocky River, Seven Hills, Strongsville, and Westlake.
