Reports to: Director of Nursing Join our dedicated team as a Pediatric Home Health Licensed Practical Nurse (LPN) and make a meaningful impact by delivering outstanding care to children in the comfort of their homes. In this role, you will assess, plan, implement, and evaluate comprehensive patient care while coordinating education for families. Key Responsibilities: Deliver expert nursing care to assigned pediatric patients. Evaluate children's physical and emotional health, home settings, and family dynamics. Educate patients and families about their rights and available care options. Monitor ongoing patient progress, implement care plans, and collaborate with physicians and clinical teams. Conduct nursing procedures as ordered by physicians, including medication and IV therapy. Oversee Home Health Aides to ensure high-quality care delivery. Assist patients with daily living activities, promoting independence and self-sufficiency. Maintain thorough and accurate clinical records and reports for each patient. Participate in on-call rotations as required. Carry out additional duties assigned by management. Supervisory Responsibilities: Lead and assess patient care in collaboration with team members. Be involved in the hiring, training, performance assessments, and addressing staff concerns. Qualifications: Current LPN license in good standing. Graduate of an accredited nursing program. One year of nursing experience preferred, but not mandatory. Skills and Abilities: Outstanding nursing, observation, communication, and leadership abilities. Knowledge of home health care and relevant regulatory standards. Good physical and mental health are crucial. Empathetic and patient-centered approach with a caring attitude. Ability to effectively address practical issues and interpret diverse instructions. Physical Requirements: Moderate physical activity, including driving to patient locations and assisting with mobility. Regularly uses hands, talks, and hears; frequently stands, walks, and bends. Must be able to lift over 100 pounds occasionally. Vision requirements include close vision, color vision, and depth perception.
