Job Description Job Title: Consumer Directed Services Specialist Department: CDS Program Reports To: Program Manager - Consumer Directed Services FLSA: Non-Exempt SUMMARY A Consumer Directed Services (CDS) Specialist assists clients in managing and directing their own personal care services, ensuring they have the resources and knowledge to make informed decisions. They help clients navigate service options, coordinate care plans, and maintain compliance with state and federal regulations. Additionally, a CDS Specialist provides ongoing support and education to clients and their families to enhance their independence and quality of life. BASIC FUNCTIONS AND GENERAL RESPONSIBILITIES: Service Coordination and Support • Provide training to participants regarding their responsibilities and requirements of the CDS program. • Assist participants as needed with technical assistance for the purpose of training the attendants regarding their responsibilities and requirements of the CDS program. • Monitor compliance of participants for CDS through monthly contacts as required. • Assist with electronic visit verification setup and troubleshooting and provide training to participants and their attendant employees as requested. • Document all contacts with participants in appropriate databases or spreadsheets, ensuring that service participant documentation is accurate, thorough, and completed in a timely manner according to agency standards, service, accreditation, and regulatory authority requirements. • Provide services in accordance with regulatory authority requirements, funding source requirements, accreditation requirements, and legal requirements. • Report suspected instances of abuse, neglect, or exploitation to the appropriate state department in accordance with agency standards and law. • Assess customer satisfaction by conducting participant satisfaction surveys or other required methods. • Effectively and efficiently use computers or mobile devices to input, access, modify, or output information. This includes the ability to quickly access, store and retrieve data. • Assist in completing at minimum TAP Demonstrations and AT demonstrations. • Document, track and report all AT and TAP demonstrations for the agency • All Other Duties as Assigned Other Departmental Requirements • Participate in special activities, work groups, committees, and other duties as assigned. • Accept and complete referrals for AT,TAPand CDS services as needed. • Communicate with supervisor and peers to ensure office coverage during regular business hours. • Provide support and backup for the front desk, as needed. • Promote a positive and open team environment. • Accept other duties and assignments as needed to enhance and maintain workflow. • Participate in organizational meetings and training as required. • Comply with applicable laws and regulations, established agency policies and procedures, and professional standards. • Maintain participant confidentiality as required by law. Function as a representative for other specialists in their absence Outreach • Participate in outreach, networking, public presentations, and other means to educate the public about the services provided by Life Unlimited. • Serve on committees, as appropriate. Physical Expectations • This position requires extended periods of walking and standing with some periods of sitting which are dependent upon the needs and activities of the individuals supported. It may require light to heavy lifting, up to fifty pounds independently, and occasional stooping, kneeling, bending, and/or climbing stairs depending on the needs of the individual supported. • The position will require occasional physical intervention if confronted by a person displaying combative or potentially dangerous behavior. Working Conditions Office/home environment with noises from appliances, telephones, and occasional small machinery (lawn mower, etc.). May be exposed to bloodborne pathogens and/or infectious diseases. Occasionally, prolonged hours or unusual chores to meet the unexpected needs of the individuals (i.e., medical and/or behavioral). QUALIFICATIONS • Relevant experience, or a bachelor's degree in social work, psychology, or a related human services field • Ability to work in a role that is largely sedentary. • Sufficient knowledge of Medicaid, Medicare, Social Security benefits, Veterans benefits, and community resources, in order to refer people with disabilities to the appropriate resources. • Ability to travel, valid driver's license, and current vehicle insurance. • Must be able to communicate effectively in English - both in writing and orally. • Must not have been convicted of any disqualifying felony and must not have been substantiated for abuse or neglect in the state of Missouri, as recorded by the Department of Mental Health's Employee Disqualification List. • Must demonstrate safe practices in all areas and exemplify positive professional standards. • Must obtain a Class E driver's license during new hire orientation- must carry minimum coverage applicable by law for auto insurance. • Must Receive Seasonal Flu Vaccination Equal Opportunity Employer
