Roles & Responsibilities - Ensure that the client's plan of care is individualized, appropriate, and executed as written. - Complete documentation for all patients transferred to an inpatient facility. - Process OASIS assessments and verify accurate start of care and episode date range. - Lead weekly case conferences and address recertification or discharge decisions. - Review orders and ensure that all documentation is accurate, including medications and medical supplies. - Follow up on patient assessments and coordinate with physicians as necessary.
