Field Nurse Supervisor

Liberty Home Care is looking for a motivated Registered Nurse to join our growing home care nursing team! Schedule: Monday - Friday 8:30am-4:30pm Duties & Responsibilities: Provide assessment of newly referred patients and assess health needs in home care setting Work with patient to create a patient centered plan of care Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family Prepare written documentation and summaries of the patient's progress as needed for the continuation of home health services Provide supervision and training of PCA/HHA staff Assist with the on-call rotation (stipend provided) Qualifications: NYS Licensed Registered Professional Nurse Minimum of Associate's Degree required One year Nursing experience required Home care experience a plus Benefits: We offer a strong benefits package that you are eligible for on your day of hire, including: Health insurance with fully paid employer option for yourself and family Company paid long-term disability insurance and life insurance 19 days of PTO time your first year with increases based on years of service Paid holidays 403b for retirement saving Dental, vision, disability and life insurance Company Pension with employer contribution after 1 year of service Generous mileage reimbursement EOE The pay range displayed on this job posting reflects the anticipated range for this position.
