Patient Care Coordinator Coram/CVS Health Join our team and make a difference in patients' lives. As a Patient Care Coordinator, you'll support new infusion patients, deliver high-quality service, and help simplify healthcare for those who need it most. What you'll do: Serve as the first point of contact for patients, caregivers, and healthcare providersanswering questions, resolving concerns, and providing education. Guide new patients through the onboarding process with empathy, professionalism, and attention to detail. Handle a high volume of phone calls, using active listening and de-escalation skills for challenging situations. Multitask and prioritize effectivelynavigating multiple computer screens and systems while capturing all necessary patient information. Work independently from home, reliably following shift, break, and lunch requirements. Collaborate with internal teams to support referrals and admissions. Required Qualifications: 2+ years' experience in healthcare or customer service, including working with insurance benefits Strong verbal and written communication, empathy, and active listeningespecially by phone Comfort with computers and Microsoft Office; able to multitask and navigate multiple screens Reliable, organized, and able to work independently from home as scheduled High school diploma or GED Preferred Qualifications: Experience with healthcare insurance and medical terminology Certified Pharmacy Technician (CPhT) Experience providing telephonic customer service in a healthcare environment Prior experience capturing information while toggling between multiple computer applications Why CVS Health?
