Job Description & Requirements Specialty: Family Practice Discipline: Nurse Practitioner Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Travel Pay Breakdown: • Hourly: $34.97/hr Taxed ($1,398.80/wk Taxed) • Per Diem Eligibility: $1,687.00/wk = • $3,085.80/wk Gross Eligibility Hours : 40hrs/wk Schedule : Monday through Friday The Family Nurse Practitioner (FNP) provides primary care to a diverse population of homeless and low-income patients across the lifespan, in a Federally Qualified Health Center. The FNP provides equitable, team-based integrated and culturally responsive patient-centered care. The FNP diagnoses, treats and manages acute and chronic health conditions, and provides preventive care focused on health risk factor reduction and patient self-management. The FNP works in a care team environment and ensures efficient, effective, and integrated patient care through joint planning and problem solving with internal and external care team members. Required : • OR RN license (NURSYS) • OR NP license • NPI number - />• DEA registration • OPDMP (Oregon Prescription Drug Monitoring Program) registration is also required.
