Company Description Get Aging, is a small physician-owned Geriatrics and Palliative Care Practice serving home-bound or home-limited older adults. We believe that the best care can be delivered outside of large corporate health care. Role Description This part-time position as a Clinician (NP, MD, or PA) focuses on providing home-based geriatric care in the San Francisco Bay Area. Primary responsibilities include performing comprehensive medical evaluations, managing chronic conditions, and collaborating with multidisciplinary care teams to deliver personalized care for older adult patients. This is an on-site role requiring travel to patients’ homes. Qualifications Active and unrestricted licensure as a Nurse Practitioner, Physician, or Physician Assistant. Board certification in a relevant field is strongly preferred. Proficiency in conducting health assessments, managing chronic diseases, and delivering high-quality patient care. Strong interpersonal skills and the ability to build trusting relationships with patients and their families. Experience and interest in Geriatrics and palliative care, or home-based healthcare is a plus. Effective communication, teamwork, and ability to operate within a multidisciplinary team.
