Nurse Practitioner Hospital At Home Per Diem The Per Diem Hospital At Home Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. Responsibilities include performing and documenting accurate, complete, and relevant history and physical assessments appropriate for the patient's age, gender, and clinical problem; ordering, performing, and interpreting laboratory tests and diagnostic studies/procedures in collaboration with physicians and consultants as appropriate; managing general medical and surgical conditions based on comprehensive knowledge of etiologies, risk factors, pathophysiology, and the indications and contraindications of pharmacologic agents and other treatment modalities; prescribing medication safely and effectively in accordance with best practices, hospital policy, and NJ regulations, providing patient education as needed; and more. Qualifications required include a graduate of an NLN/AACN accredited nursing program, a master's or doctorate degree from an accredited Nurse Practitioner program, maintaining clinical competencies through continuing education and clinical practice, experience in a relevant patient care setting, national provider identifier, prescriptive authority through CDS and DEA registration according to New Jersey state law, and more.
