Home Health Physical Therapist - Zion, IL Join our client's exceptional home health team as a physical therapist and bring your skills directly to patients' homes. You'll assess and implement customized therapy plans to help patients improve mobility, independence, and quality of life. Responsibilities: Provide in-home PT assessments and create personalized treatment programs Monitor and document progress in coordination with the care team Educate patients and families on therapy routines and safe equipment use Supervise PT assistants and collaborate with other providers Schedule: Monday-Friday, flexible daytime hours 1 weekend every 6 weeks and 2 holidays annually Travel: Local travel required throughout Zion and nearby areas.
