Position Details: Main Office location: Concord, MA Service Area: Concord, MA and Surrounding Areas Job Type: Per Diem Schedule: Flexible Hourly Rate: $25.00 Responsibilities: Provide personal care assistance such as bathing, grooming, dressing, and mobility support Observe and report changes in patients' condition to the supervising nurse or therapist Document care provided and patient observations accurately Follow the care plan established by the Registered Nurse Maintain a safe and supportive environment for patients in their homes Promote quality, continuity, and safety of patient care Qualifications: Certified Nursing Assistant (CNA) in Massachusetts preferred; Home Health Aide certification accepted Ability to read and follow written care plans and document services provided Ability to work independently with minimal supervision Compassion and empathy for individuals who are ill, injured, or elderly Current CPR certification Strong communication and interpersonal skills Valid driver's license and reliable, insured vehicle Pathways Healthcare is committed to providing exceptional care to our patients and fostering a positive work environment for our team members.
