Licensed Practical Nurse (LPN) - Per Diem - Goshen, NYStar Nursing is seeking LPNs for urgent per diem opportunities at a Sub-Acute Rehab Facility in Goshen, NY (10924). Full-time block booking is available, offering the opportunity to secure a steady schedule and potentially exceed 40 hours per week. Job Details: Location: Goshen, NY Facility: Sub-Acute Rehab Position: Licensed Practical Nurse (LPN) Job Type: Per Diem Shifts Available: Overnight (11 PM - 7 AM) PM Shift (3 PM - 11 PM) AM & PM Weekend Shifts Pay Rates: Per Diem Rate: $42 per hour (taxable) Split Pay Rate: $20 per hour (taxable) + $960 (tax-free) = $1,760/40 hours Job Responsibilities: Provide direct nursing care under RN or physician supervision Monitor and document vital signs Observe and report changes in patient conditions Assist with wound care, dressing changes, and other medical treatments Administer prescribed medications and monitor for adverse reactions Assist patients with mobility and personal hygiene tasks Maintain accurate and up-to-date patient records Ensure compliance with facility policies and infection control protocols Requirements:Step 1: Resume with long-term care experience Active LPN license Current BLS/CPR certification Skills checklist 1-2 recent supervisor references Step 2 (Upon Offer): Physical and PPD (within the last 12 months) MMR, Varicella, Hep B (or decline) Tdap/Flu (or decline) COVID negative test (current) LGBT Training Certification Background check and urine drug screen Copy of driver's license and Social Security card If you meet the above requirements, please submit your resume to !
