LPN/LVN (License Practical Nurse)

Job Title: Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN)Location: 380 Wray Large Rd, Jefferson Hills, PA 15025 Unit Type: Long-Term Care / Skilled Nursing Facility Division: Nursing Care Setting: Post Acute Shift Length: 12 Hours Guaranteed Hours: 36 hours per week Years of Experience Required: 1 Year Required Certifications: Basic Life Support (BLS) Charting System: Point Click Care (PCC) for assessments and nurse notes (Medication not tracked electronically) Unit Notes: Facility Type: Free-standing Assisted Living Facility with Memory Care units; some Independent Living; Memory Care units are secured and locked. Bed/Resident Count: Varies by location, primarily private rooms. Staffing Ratios: RN: 1:40 on day shift; responsible for overseeing nurse aides LPN: 1:40 on all shifts (day, evening, night) CNA: 5 on days, 4 on evenings, 2-3 on nights Common Diagnoses / Types of Patients: Predominantly memory care including dementia and Alzheimer's Job Duties: Administer medications and treatments as per state regulations (IVs, enteral feedings, ostomies, trachs are rare) Provide basic nursing care and support Monitor patient health and report changes Assist with daily living activities and coordinate care with other healthcare providers Maintain accurate and timely documentation in PCC Follow facility protocols and regulations, especially regarding dementia care Experience or 'Must Have' Skills: Experience in dementia care required Ability to work independently and make decisions when RN is not on-site Basic knowledge of medication administration and patient care Strong communication skills Additional Information: Physician Availability After Hours: Available via phone or telehealth in some markets Rehabilitation Services: Contracted PT/OT/SLP, Pulmonary or Cardiac Rehab, Case Managers, Social Workers, Dieticians Oxygen Therapy/Equipment: Rare use of CPAP/BIPAP; High Flow Oxygen not used Shift & Scheduling: Day Shift: 7 AM - 3 PM Evening Shift: 3 PM - 11 PM Night Shift: 11 PM - 7 AM Weekend Requirement: Every other weekend
