Assignment Details: - Contract Length: 14 Weeks Requirements: - NY LPN Licensure - BLS (certification must be through AHA/RQI) - 1 year experience as LPN Roles & Responsibilities: - Collects data relevant to residents' physical and psychosocial conditions on an ongoing basis and communicates findings to the RN and other members of the Interdisciplinary Team. - Contributes to the creation of a developmentally age-appropriate and culturally relevant plan of care reflecting nursing diagnoses and interventions to achieve desired outcomes, involving the resident/family and healthcare team. - Recommends revisions to the plan of care after evaluating resident outcomes and indicates the need for referrals to ensure all aspects of the resident's care have been addressed. - Collaborates with the resident/family and members of the Interdisciplinary Team, ensuring customer satisfaction. - Maintains current knowledge and competency in nursing practice and participates in mentoring/orientation of staff. - Provides leadership in the professional practice setting under the direction of the RN. Compliance / Notes: - Occupational exposure to bloodborne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
