You will be the primary contact for all disciplines involved in patient care, managing visit frequencies throughout the patient’s care episode. Key responsibilities include performing accurate OASIS data collection, maintaining updated medication profiles, reporting lab values to physicians promptly, and participating in weekly case conferences and monthly case management meetings.
