Position: Clinical Manager - Hospice RN Location: Middlebury, CT Employment Type: Full-Time Salary: $100,000 - $115,000/year Schedule: Monday to Friday, 8 AM - 5 PM Why Join Us? Qualifications: Preferred: Associate degree Preferred: Certified Hospice Nurse (CHPN) Minimum 3 years of experience in hospice, home health, or related nursing fields At least 1 year of supervisory experience is required Valid CPR certification preferred Must be a licensed driver capable of traveling to all business locations Compliance with state regulations and requirements Your Role Includes: Supervising patient and family care team members, collaborating with leadership Managing scheduling, payroll, and relevant administrative tasks Ensuring timely assessments and documentation are complete by qualified team members Overseeing the Interdisciplinary Group (IDG) in creating and reviewing care plans Monitoring and mentoring staff in patient care and documentation practices Facilitating IDG meetings and providing ongoing support Participating in quality improvement activities Fostering open communication internally and externally Our Commitment to You: We recognize that caring for others begins with caring for you.
