Job Description & Requirements Specialty: Master of Social Work Discipline: Social Work Duration: 8 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Position Overview: We are seeking a compassionate Medical Social Worker (MSW) to provide clinical social work services, counseling, discharge planning, and case management to hospital patients and their families. The Social Worker will collaborate with the interdisciplinary healthcare team to assess psychosocial needs, develop care plans, and support patients across the continuum of care. Key Responsibilities Conduct psychosocial assessments to identify emotional, social, financial, and environmental needs of patients and families. Provide clinical counseling, case management, and discharge planning. Develop and implement individualized psychosocial plans of care based on patient strengths and needs. Collaborate with physicians, nurses, and interdisciplinary teams to coordinate patient care. Monitor patient progress toward goals and identify barriers impacting length of stay or discharge planning. Facilitate referrals and integration with community resources and support services. Provide support to patients and families coping with illness, grief, or loss. Maintain accurate and timely documentation in the patient medical record and electronic systems (Cerner, MIDAS+). Participate in quality improvement initiatives, clinical practice teams, and community networking activities. Ensure compliance with hospital policies, regulatory standards, and organizational values. Required Skills Strong psychosocial assessment and counseling skills. Excellent critical thinking and clinical decision-making abilities. Experience supporting patients and families dealing with grief, loss, and complex care needs. Strong organizational, communication, and documentation skills. Ability to work effectively with diverse patient populations (neonatal to geriatric). Basic proficiency with word processing and electronic medical record systems. Education & Licensure Master of Social Work (MSW) from an accredited program (CSWE). Licensure preferred but not required. Physical Requirements Ability to stand, walk, bend, and sit for extended periods during the workday.
