Reports to: Director of Nursing Join our dedicated team as a Pediatric Home Health Registered Nurse, where you will play a crucial role in providing exceptional care to children and their families in the comfort of their homes. As a valued member of our community health service, you will utilize your nursing expertise to develop and implement personalized care plans that address the unique needs of each patient. Key Responsibilities Conduct comprehensive initial assessments of patients and their families to evaluate home care needs. Regularly review and re-assess patient nursing needs to ensure appropriate care delivery. Develop and initiate personalized care plans that evolve with the patient's condition. Utilize health assessment data to identify nursing diagnoses and formulate actionable care plans. Administer prescribed medications and treatments, and guide patients and families in managing health care effectively. Identify discharge planning needs early in the care process to facilitate smooth transition from home care. Act as the Case Manager for assigned patients, coordinating all aspects of their care. Communication Duties Maintain accurate clinical notes and provide necessary updates to the primary physician every 60 days. Collaborate with physicians regarding patient needs and any changes in condition; ensure timely updates on care plans. Engage with community health resources to ensure holistic care for patients. Additional Functions Participate in on-call duties as defined in the policy. Ensure that necessary equipment and services are arranged for each patient. Supervise and evaluate the care provided by home health aides every two weeks. Qualifications Graduate of an accredited nursing program with 1-2 years of recent acute care experience. Current nursing license and CPR certification. Bachelor's degree preferred, with at least one year of experience in home health care. Effective communication skills and proficiency in nursing practices. Physical ability to perform nursing tasks, including lifting and transferring patients. Must have a valid driver's license and ensure your vehicle meets state and organizational requirements.
