Position Details: Join the Hartford Hospital's Department of Medicine as an Advanced Practice Provider! We are looking for a dedicated non-clinical Nurse Practitioner or Physician Assistant to play a vital role in engaging with complex patients after their discharge. Your efforts will help ensure patients follow discharge instructions, manage their medication prescriptions, and secure follow-up appointments with their healthcare providers. Role: Open to both Physician Assistants and Acute Care Nurse Practitioners. Role Scope: Your primary goal will be to reduce readmission rates by proactively engaging with high-risk patients post-discharge. You will utilize an Epic AI model to identify these patients and coordinate their necessary follow-up care.
