Position Details: Work Shift: Monday - Friday, 8:00 am to 5:00 pm CST with rotating on-call responsibilities. As a Nurse Practitioner, you will join a dedicated team collaborating with physicians, patients, and families to develop personalized care plans that address individual health needs. Your expertise will provide essential clinical and non-clinical support to enhance patient well-being. Key Responsibilities: Deliver primary and urgent healthcare via telephonic and video consultations for patients receiving home and community-based services through state Medicaid programs. Develop and implement comprehensive clinical care plans to manage complex issues in adult patients, addressing co-morbid conditions and resource limitations. Conduct detailed patient assessments, including history taking and physical examinations, to create effective treatment plans. Identify and address care gaps with strategic solutions. Support the physical and psychosocial needs of patients and their families in collaboration with a multidisciplinary team. Educate patients and families on medications, chronic illness management, dietary requirements, and preventive strategies. Coordinate care with other healthcare providers and maintain accurate medical records in compliance with regulations. Engage in continuing education as required by certification mandates. Prescribe medications as permitted by state guidelines. Minimum Qualifications: Master's Degree in Nursing. Current national NP certification. Active Family or Adult NP license in Texas. Valid RN Compact license. 2+ years of experience managing complex patient cases. Familiarity with Electronic Medical Records (EMR). Preferred Qualifications: DEA registration or eligibility. Active Medicaid number in Texas is highly preferred. Experience caring for patients with developmental disabilities or chronic illnesses is strongly preferred.
