Exciting Opportunity for a Case Manager - Registered NurseJob Description:- Seeking a passionate Registered Nurse specializing in Case Management to join our dynamic team! -Shift:Day 5x8-Hour (08:00 - 16:30) -Experience Required:- 2 years in acute care case management/discharge planning - Travel experience preferred (1st time travelers may be screened) -Charting System:Proficient in Epic -Patient Care:- Manage a daily census with a case load of up to 25 patients - Patient Ratios: Days 1:20 - Weekend Coverage: Every other weekend, must be available for holidays -Diverse Patient Age Groups:- Neonates, Adolescents, Infants, Adults, Pediatrics, Geriatrics -Collaborative Interdisciplinary Support:- Work alongside IV Teams, Physical Therapy, Respiratory Services, Social Services, and more -Key Responsibilities:- Develop and coordinate care plans - Conduct needs assessments and manage discharge planning - Familiarity with regulatory guidelines including Workers Compensation and CMS - Knowledge of admission criteria and InterQual guidelines is highly preferred -Required Skills:- Competency in assessing patients and care coordination - Arrange DME, SNF, and ambulance transport independently - Ability to facilitate family meetings and participate in treatment plan development - Experience with diverse health coverage plans including Medicare and MediCAL -Orientation:40 hours of initial training to set you up for success -Computer Skills:Proficiency in computerized documentation is essentialWhy Catalytic Solutions (CatSol):At CatSol, we connect dedicated therapists with rewarding assignments across the country.
