Exciting Opportunity for a Case Manager - Registered Nurse!Job Description- Join a dynamic healthcare team as a Case Manager, specializing in acute care! -Start Date:ASAP -Shift:Day 5x8-Hour (08:00 - 16:30) -Patient Ratios:1:20 -Years of Experience required:- Minimum of 2 years of current acute care hospital inpatient Case Manager experience. - Experience in Care Coordination and Admission Reviews within the last 6 months. -Travel Experience:- Yes, seasoned traveler required. -Charting System:- Must be proficient in Epic. -Essential Skills:- Knowledge of InterQual Criteria. - Experienced in: - Care Coordination - Discharge Planning - Needs Assessment/Order DME - Prior Authorizations - Experience with HMOs, IPAs, etc. -Patient Demographics:- Adults and Geriatrics -Daily Census:- Manage a caseload of 20-22 patients. -Weekend Coverage:- Required to work every other weekend with at least 2 weekend shifts in a 4-week schedule to meet department needs. -Interdisciplinary Support:- Collaborate with Social Services. -Special Procedures/Unit Details:- Admission Criteria (InterQual) - Assessing patients and healthcare delivery coordination. -Dress Code:- Business casual with a lab coat or dark solid scrubs with lab coat. -License Requirements:- Current California state license required. - Pending licenses accepted. -Pre-Approved Time Off:- One request allowed. -RTO Restrictions:- No more than 7 days.Why Catalytic Solutions (CatSol):At CatSol, we connect dedicated therapists with rewarding assignments across the country.
