Your Impact & Responsibilities: Clinical Oversight: Lead the nursing care for a designated group of residents, ensuring all medical treatments and medications are administered accurately. Acute Assessment: Perform high-level clinical assessments, identifying changes in condition and coordinating immediately with physicians and the interdisciplinary team. Care Coordination: Manage the admission and discharge process for short-term rehab patients, ensuring seamless transitions of care. Team Leadership: Supervise LVNs and CNAs, providing clinical guidance and ensuring facility protocols are followed. Documentation: Maintain precise, real-time electronic medical records in compliance with state and federal regulations. Qualifications: Licensure: Must possess a current, valid California Registered Nurse (RN) license. Certification: Current BLS/CPR certification. Experience: Previous experience in post-acute, SNF, or acute care is preferred.
