Job Title: Registered Nurse (RN) - Case Management Location: New York, NY Duration: 14 Weeks Contract Schedule: 5 Days/Week | 40 Hours/Week Shift: 9:00 AM - 5:30 PM Pay Rate: $58 - $65 per hour Position Overview "Navitas Healthcare, LLC" is seeking an experienced Registered Nurse (RN) - Case Management to coordinate patient care, manage healthcare resources, and support utilization management to ensure appropriate and cost-effective treatment. The RN will collaborate with multidisciplinary teams to monitor patient progress, adjust care plans, and facilitate safe discharge planning. Responsibilities Coordinate and manage patient care plans for optimal outcomes Perform case management and utilization review for medical necessity Collaborate with physicians, nurses, and care teams Monitor patient progress and update care plans as needed Coordinate discharge planning and post-acute care services Maintain accurate documentation and ensure regulatory compliance Required Qualifications Active New York State RN License Minimum 2 years of case management, resource management, or utilization management experience BLS Certification (AHA) required ACLS Certification (AHA) required for Queens locations Strong care coordination and communication skills For more details contact at pmohanty@ or Call / Text at 516-806-4227.
