Description The Inpatient Registered Nurse (RN) Care Manager plays a vital role in delivering comprehensive, patient-centered care management services for at-risk patients within an acute care setting. This position involves key responsibilities such as patient assessment and planning, care coordination, resource utilization management, discharge planning, thorough documentation of interventions, ensuring regulatory compliance, and advocating for patients. The primary goal of the Inpatient Care Management RN is to guarantee that the right care is administered at the right time and in the right setting, optimizing the use of healthcare resources across the continuum of care. Required Qualifications: Associate's Degree in Nursing upon hire California Registered Nurse License upon hire 2 years of acute care experience in a Medical Surgical or Inpatient setting 1 year of experience in care management or utilization review in any setting, OR Upon hire, successful completion of TIPS program or Case Management Orientation Program Preferred Qualifications: Bachelor's Degree in Nursing or higher obtained within 3 years of hire Case management certification (ACM or CCM) Why Join Providence? Posted are the minimum and maximum wage rates for this position. The successful candidate's placement within the wage range will be influenced by relevant experience and other applicable factors. This range reflects base pay; additional compensation may be available, including shift differentials, standby/on-call pay, overtime, premiums, extra shift incentives, or bonuses.
